The 4-axle variant of the HVY Mixer is a large cement mixer appearing in Grand Theft Auto V and Grand Theft Auto Online, and due to appear in Grand Theft Auto VI.

The Mixer briefly appeared in the September 2022 GTA VI leaks in a single clip, seen driving northbound in red past the Gas Stop branch in Port Gellhorn.[1] The truck's base variant also appeared in leaked footage.

Based on the Biff, the Mixer's cabin and hood design closely resemble a Kenworth T800.

The 4-axle variant of the Mixer assumes the cab design of a Biff, but with rounded-over front fenders and dual circular headlights mounted to the sides of the hood, as opposed to being embedded into the fenders. The chassis retains the triple-axle design of the Biff, as well as the other variant of the Mixer, but with an additional 4th "tag axle" mounted on the rear of the truck, designed to provide stability, however it is unusable in-game.
In comparison to the smaller Mixer, the 4-axle Mixer is slightly bigger and consists of only a singular color on both the cab and the mixer drum itself.

Despite having a separate handling entry in the game files, the larger 4-axle variant of the Mixer performs identically. It is a large, slow industrial vehicle with average acceleration and top speed. The mass of the vehicle allows it to ram other vehicles out of the way, but it isn't completely unstoppable.
The truck features a turbocharged inline-4 engine under the hood, powering the rear wheels through a 4-speed gearbox. The engine sound is shared with trucks like the Phantom and the Rubble.

- One can be found parked near a house in Sandy Shores, only a block north-west from Trevor's Trailer.
Both Mixers are part of the vehiclemodelsets, "CARS_CONSTRUCTION" and "BUILDING_SITE_TRUCKS". Both sets of vehicles commonly appear driving in and out of construction sites or parked around them.
