The 4-axle variant of the Mixer assumes the cab design of a Biff, but with rounded-over front fenders and dual circular headlights mounted to the sides of the hood, as opposed to being embedded into the fenders. The chassis retains the triple-axle design of the Biff, as well as the other variant of the Mixer, but with an additional 4th "tag axle" mounted on the rear of the truck, designed to provide stability, however it is unusable in-game.
In comparison to the smaller Mixer, the 4-axle Mixer is slightly bigger and consists of only a singular color on both the cab and the mixer drum itself.

Despite having a separate handling entry in the game files, the larger 4-axle variant of the Mixer performs identically. It is a large, slow industrial vehicle with average acceleration and top speed. The mass of the vehicle allows it to ram other vehicles out of the way, but it isn't completely unstoppable.
The truck features a turbocharged inline-4 engine under the hood, powering the rear wheels through a 4-speed gearbox. The engine sound is shared with trucks like the Phantom and the Rubble.
