There are several clothes which can be worn that increase CJ's sex appeal massively. These are things like suits, or tank tops which show off his muscular arms. Because of the latter point, reaching maximum strength will also increase sex appeal significantly. Driving high-end cars helps too.

The achievement is twofold, as it is both an achievement/trophy on supported consoles, as well as unlocked in the Social Club.
